REQUEST FOR QUOTATIONS

TO WHOM IT MAY CONCERN:
Marshall Municipal Utilities of the City of Marshall, Minnesota invites your written quotation until 1:00 p.m. on Thursday, October 13, 2011 for the following:
Basic and Supplemental Group Life Insurance and Long Term Disability Insurance
Quotations shall be made on the proposal forms provided by Flexible Benefit Consulting and be submitted to Marshall Municipal Utilities via US mail in a securely sealed wrapper and clearly marked upon the outside indicating Insurance RFP for Marshall Municipal Utilities. Two (2) copies of the quotations shall be directed to:

Quotations shall be in accordance with the specification information now on file at the office of Flexible Benefit Consulting. Request for proposal forms and specification information can be obtained by calling Justin Kroeger of Flexible Benefit Consulting at 320-864-3525. Justin can also be reached via e-mail at Justin@flexiblebenefitconsulting.com.
Quotations will be received until 1:00 p.m. on Thursday, October 13, 2011 at the office of Marshall Municipal Utilities. Quoted price must be valid for a minimum of 60 days after the scheduled closing time for the receipt of quotations, unless otherwise noted.
Marshall Municipal Utilities reserves the right to reject any or all proposals, waive informalities, and award the contract based on its best interest.
